Share your child's ASQ results securely and conveniently with your child's medical provider via a confidential online portal.

Ages and Stages Questionnaires (ASQs) are a key part of your child’s standard well child exams from ages 2 months through 5 years.  We encourage parents to complete and submit these age-appropriate developmental screening questionnaires through the secure link below.  If this is done a few days before your appointment, we can better serve you when you arrive in our clinic!

Ptarmigan Connections offers patient centered behavioral and developmental pediatric services to the Mat-Su Valley Region. Our team is dedicated to supporting patients, and their families, with best practices in medicine and science, as well as educational support. We have an interdisciplinary team of professionals that include a pediatrician, psychologists, speech-language pathologists, counselors and psychiatric nurse practitioners.

What does a Developmental and Behavioral Pediatric Clinic do?

Specialized health care professionals are necessary when a baby, child, or adolescent has behavioral or developmental needs that go beyond the scope of “typical” health issues that are common for their age. Ptarmigan Connections, based out of Wasilla, Alaska, exists to support the child’s family, and his or her healthcare provider to address these concerns. We combine skilled professionals and helpful resources to offer physical, social-emotional, behavioral, and developmental support for children and their families. If you have any concerns about your child’s development in any of the aforementioned areas, please ask your child’s primary care provider about a referral to Ptarmigan Connections.

You may benefit from having your child see a Ptarmigan Connections professional if your child has (or you think he or she may have):

• Delayed speech and/or trouble understanding language
• Delayed motor skills (crawling, walking, eating, riding a bicycle)
• Poor social skills
• Trouble sleeping (including bedwetting)
• Trouble feeding or eating
• Sensory sensitivities
• Trouble at school (paying attention; learning to read, write, or do math)
• Cerebral Palsy
• Attention Deficit Hyperactivity Disorder (ADHD) (with or without hyperactivity)
• Learning disabilities
• Anxiety disorder
• Depression
• Tics or Tourette Syndrome
• Spina Bifida
• Autism Spectrum Disorder
• Intellectual disability
• Other chronic conditions, serious illnesses, or complications due to prematurity
